The Link Worker will support the delivery of the social prescribing research project in the Brecon and District area. They will meet clients referred by primary care and help them to access more appropriate services and support like welfare services, well-being activities, non-clinical interventions such as mindfulness and stress control, or peer support groups. The focus will be on empowering the client to make their own choices and to set personal goals. The Link Worker will need to build good relationships with their local primary care teams, as well as develop an in-depth knowledge of local services and activities.
As this is a research project, the Link Worker will support the collection of client information so that the effectiveness of the service can be evaluated. The link worker will also be involved in the design phase, working collaboratively with other Link Workers, Mind Cymru and other partners to complete detailed mapping work of services and referral pathways in your locality to inform the final model.
The Link Worker will be expected to travel between local sites on a regular basis and to attend occasional national project meetings across Wales as required.
