The Partnership Forum for Older People in Powys currently recruiting up to 5 older people / service users representatives to join the Forum to contribute to all issues affecting older people such as housing, transport, leisure, health and community issues.
The Partnership Forum for Older People is looking for 5 representatives of older people / service users from a cross section of the community across Powys to contribute ideas and opinions that will influence decisions about how Adult Care Services (and Health) in Powys County Council spend money available and on what services, for the benefit of older people.
As a representative you will provide vital input into the decision making process around commissioning of services, a chance to influence policy, service design and delivery of services. ensuring that services that are provided meet older people’s needs. The Forum via its members and Joint Chairs will ensure that these issues are fed through the appropriate corporate planning and service groups including Regional Partnership Board, Ageing Well Partnership Board and Senior Management Team meetings for action and/or comment.
You will be required to attend up to 6 Partnership Forums meetings a year, and to help gather the experiences and stories of other people to bring to these meetings. The Partnership Forum is co-chaired by a manager from Adult Social Care and an older person representative. Membership of the Forum includes service users, older people, a representative from Powys (t)Health Board, a representative from Powys Age Cymru and other 3rd sector organisations. Specialist speakers are brought in to discuss topics that are relevant and inform members of the group about current and new services that the Local Authority and Health provides.

Mae’r Fforwm Bartneriaeth ar gyfer Pobl Hŷn yn chwilio am 5 cynrychiolydd ar ran pobl hŷn / defnyddwyr gwasanaethau o groestoriad o’r gymuned ar draws Powys i gyflwyno syniadau a barn a fydd yn dylanwadu ar benderfyniadau ar sut y bydd Gwasanaethau Gofal i Oedolion (ac Iechyd) Cyngor Sir Powys yn gwario’r arian sydd ar gael ac ar ba wasanaethau er budd pobl hŷn.
Fel cynrychiolydd, byddwch yn cyfrannu at y broses o wneud penderfyniadau ar gomisiynu gwasanaethau, cyfle i ddylanwadu ar bolisiau, dylunio gwasanaethau a chyflwyno gwasanaethau, gan sicrhau bod y gwasanaethau hynny’n ateb anghenion pobl hŷn. Bydd y Fforwm, trwy ei aelodau a’r Cyd-gadeiryddion, yn sicrhau bod y pynciau hyn yn cael eu bwydo trwy’r grwpiau gwasanaethau a’r cynllunio corfforaethol priodol gan gynnwys cyfarfodydd y Bwrdd Partneriaeth Rhanbarthol, Bwrdd Partneriaeth Heneiddio’n Dda a’r Uwch-dîm Rheoli er sylw / gweithredu.
Bydd angen i chi fynychu hyd at 6 chyfarfod y flwyddyn ac i helpu i gasglu profiadau a straeon pobl eraill i’w cyflwyno i’r cyfarfodydd hyn. Mae’r Fforwm Bartneriaeth yn cael ei chadeirio ar y cyd gan reolwr Gofal Cymdeithasol i Oedolion a chynrychiolydd pobl hŷn. Bydd aelodau’r Fforwm yn cynnwys defnyddwyr y gwasanaeth, pobl hŷn, cynrychiolydd o Fwrdd Iechyd Powys, cynrychiolydd o Powys Age Cymru ac o sefydliadau eraill y trydydd sector.
Byddwn yn trefnu siaradwyr arbenigol i drafod pynciau sy’n berthnasol a rhoi gwybod i aelodau’r grŵp pa wasanaethau presennol a newydd sy’n cael eu cynnig gan yr Awdurdod Lleol a’r Bwrdd Iechyd.
